On behalf of the Change Foundation, Ipsos Reid is conducting a survey with health care professionals and health care providers in West Northumberland to better understand your experience working with patients as they transition through Ontario's health care system.
This survey will take approximately 15-20 minutes. All of the information collected from this survey will be kept confidential. Responses will be grouped together and we will not associate responses with a specific individual.
